Basic Life Insurance
This is a voluntary group plan of term life insurance that allows you to purchase insurance equal to 1X your base annual salary not to exceed $100,000
Beneficiary
The person(s) who receive(s) the life insurance proceeds upon the death of the insured. There are two different types of beneficiaries. Primary beneficiary is the individual who will receive the life insurance benefit in the event of the covered person's death. More than one primary beneficiary can be designated to receive benefits provided you specify the percentage of your life insurance (share) for each beneficiary.
Contingent beneficiary is the individual who will receive the life insurance benefit if the Primary beneficiary is not living at the time of your death. More than one contingent beneficiary can be designated to receive benefits provided you specify the percentage of your life insurance (share) for each beneficiary.
Certificate
Document that describes the benefits that are available to the insured employee and/or their dependents.
Conversion Option
If your Group Life coverage terminates, you may elect to convert your group term coverage to a permanent policy of individual insurance without evidence of insurability.
Coverage Amount
The amount stated on the face of the certificate that will be paid in case of death.
Dependent Life Insurance
A group plan of term life insurance that allows you to purchase life insurance coverage for your spouse and/or dependent children based on a choice of specified amounts. Dependent Life lnsurance lets you make the decision on the appropriate coverage level that will meet your family's particular needs. You must elect Basic Life to elect Dependent Life coverage.
Disability Provisions
Benefit that continues insurance coverage if you become disabled.
Grandfathered Coverage
Employee and/or dependent insurance amounts that are available without providing evidence of insurability.
Increments
The multiples or amounts of insurance coverage that you can select.
Insured or Insured Life
The person whose life the certificate covers.
Medical Evidence of Insurability (MEOI) Level
Amount of insurance that can be obtained without answering medical questions.

This valuable service covers you and your dependents for medical, pre-travel and financial assistance services, 24 hours a day, 365 days a year, while traveling internationally or domestically more than 100 miles from home. You will have access to more than 600,000 pre-qualified providers in more than 238 countries. The medical assistance services include physician/hospital/dental referrals; hospital admission guarantee; medical monitoring; medical evacuation and repatriations; emergency prescription transfer; emergency message transmission; and transportation to join patient. Travel and financial services include general travel information about visa, passport, inoculation requirements and local customs; legal referrals; emergency cash/bail assistance; telephone interpretation; lost document and lost luggage assistance.

Personal Accidental Death and Dismemberment Insurance
This insurance is offered in conjunction with Basic Term Life Insurance. It protects you should you suffer an accident that results in paralysis or the loss of limb, speech, hearing or sight. If you should suffer a fatal accident, benefits are paid to your beneficiary.

Portability Option
The option of taking your coverage with you if you leave your current job or retire. You'll be able to continue your existing amount of coverage or a lesser amount depending on your needs at the time.
Principal Amount
The total amount of insurance payable under AD&D benefits.
Term Life Insurance
Provides insurance coverage at the lowest current cost by only insuring for the possibility of death during a specified period of time. Term life insurance has no cash, loan or paid-up value and no guarantees of permanence.
Total Control Account ®(TCA)
Claims are usually paid through the convenience of our TCA, which gives your beneficiary(ies) immediate access to insurance proceeds. The account earns interest on the proceeds at competitive money market rates and is fully guaranteed by MetLife. The beneficiary(ies) receives a checkbook and monthly statements and has free checkwriting privileges. There are no maintenance fees. A special toll-free customer service line handles all questions and requests. All benefit amounts of $7,500 or more are paid out through a TCA.
Voluntary Accidental Death and Dismemberment Insurance
This insurance can be written as a stand-alone product or in conjunction with any life and/or AD&D coverage. It protects you should you suffer an accident that results in paralysis or the loss of limb, speech, hearing or sight. If you should suffer a fatal accident, benefits are paid to your beneficiary. Higher amounts and family coverage are usually available.
